A care coordinator is required to join an existing team and assist the Homecare Manager in the day-to-day running of domiciliary care services at our branch located within Hackney/ Islington border. Duties will include attending reviews and setting up new personalised care packages and undertaking day-to-day communication with other coordinators, domiciliary care / support workers and service users. The post-holder will be responsible for managing a given caseload, coordination of care and support for clients in their own homes and provide mentoring and support to care workers. Key skills include ability to undertake the following: Effective care planning and risk assessments; Liaison with Social Workers, District Nurses, Doctors and other stakeholders; Participation in the recruitment, selection and training of staff (when necessary); in line with company policies, ensure workers deliver a consistently high quality of service. Duties include the accurate production of weekly work schedule for staff using in-house I.T. rostering software and meeting weekly performance targets. Essential Candidate Attributes: – Min 1 year experience in home care as a supervisor / coordinator. NVQ Level 3 in Social Care (or equivalent) desireable. Strong candidates without NVQ 3 will be considered with the expectation that they complet NVQ3 within 12months. Good Literacy skills (including report writing); Computer Literacy and experience of a rostering system (use of Electronic Call Monitoring experience an advantage). – Salary £18K to £20k per annum; depending on skills and experience. Immediate start within 4weeks of interview required, if successful. Clear CRB check is essential.
