Location : Feltham Hill, Feltham (TW13), TW13 7EQ Salary : From £16,000 to £22,000 per annum ,negotiable Company : Avant Healthcare Services Ltd Avant Healthcare Services Ltd are looking to recruit a full time Care Coordinator / Field Based Supervisor for West London Area and the following locations: Hillingdon, Ealing, Hounslow, Feltham, Southall, Isleworth, Brentford Must live within 6 miles of W4, W3, W5, W13, W12, NW10, UB1, UB2, UB5, UB6, UB2, UB3, UB7, UB8, UB9, UB10, UB11, UB18, HA4, HA5T W13, TW14, TW7, TW8, TW3, TW5 Avant Healthcare Services Ltd support people in their own home so that our clients can remain within the comfort of their own homes independently. We provide our clients with: Personal Care – Help clients with getting up and getting ready for the, washing, bathing, feeding and toileting. Practical Support such as laundry, cleaning, shopping, preparing and cooking meals and taking people out to the shops, doctors or social events. Specialised Care – Supporting people with specialist support such as end of life care, Learning Disabilities, Mental Health etc. If you have a passion about helping people maintain independence at home, and have excellent communications skills we would like to meet you. Purpose of post To ensure the service user receive a high standard of care and support whilst maintaining their independence and dignity. This role is mainly concerned with booking each service user visit with a suitably trained care/support worker at the correct time and for the correct length of time. It is key that continuity of care is followed for every service user. This role works alongside the New Service User Manager and the Field based Manager. Key tasks: • Manage the call times for all care/support staff and service users using the computerised rostering system • Manage the computer rostering system and ensure it is continually up to date • When booking a care/support worker for a call , ensure they are properly trained to carry out the role and that they have the correct travel time • Ensure the roster has minimal travel time in between calls. Aim to devise rosters which can be walked around to promote “a healthy and environmentally friendly ” roster. • Maintain up to date care plans for each service user within the computer system • Informing all service users of any changes that occur to their package of care/support in a timely way and ensure this is logged • Ensure a Field based Manager accompanies any care/support worker to a new service user • Carry out telephone checks at least monthly to every service user to ensure their service is at a high standard. Where there are issues, resolving these quickly and effectively in line with the complaint/issues procedure • Ensuring the telephone is answered within three rings and dealing with all telephone enquiries in an enthusiastic way. It is essential to be positive with everyone. • Ensure that when calls are covered for holiday,sickness or an emergency that this is done in line with our policy. Every service user visit needs to be treated with respect and it is vital to ensure that service user is aware of any changes. • Respect the environment and the company’s resources in the way you work • Adhere to all company’s policies and procedures Experience in Coordinating/Filed Supervision is desirable.This is a fantastic opportunity for a switched on , enthusiastic , reliable individual with a ‘can do’ attitude.
